This section should contain:
 
1. Simple floor plan noting:

  • Width & length of room
  • Whether doors open in or out
  • Heights of bed, favorite chair (arms present) and couch
  • Width of doors (inside of frame)
  • Flooring (wood, vinyl, carpet, tile) & presence of doorsill
  • Lighting sources (overhead, lamps) and electrical outlets

It may be helpful to do this on graph paper to scale 1 square = ¼ foot.
 
2. Photos of room, especially entry/exit to other rooms, halls, kitchen, etc.
 
3. Recommendation for Bedroom

  • Bed should allow access from the wheelchair.
  • Install a transfer handle on bed frame if needed to allow individual to grab for transfer assistance; mount a trapeze bar or grab handle in the wall above bed to assist with scooting up in bed or rolling.
  • Light source, clock, phone with large flat numbers for dialing & emergency speed dialing numbers and any time up call system should be arranged on a bedside tabled. All electronic controls for TV, VCR, stereo should be within reach; velcro to headboard or side of nightstand.

If the individual will require a work-station for computer, art work etc, an assistive technologist and/or a recreational therapist may be needed to create and or simulate a work space.
