Every time you go to a doctor or facility or call the doctor or facility, make an entry in this section.
Write everything. If possible, the patient should give this job to someone else to do – wife, mom, friend. The “recorder’s” job is to record all information given. Don’t try to process it or ask questions at this time. Just write. This record will provide a pattern that will help you recognize what’s going on with you (or your loved one). It will also bring back to your mind all the things you missed that the doctor said – meds, reactions, next visit time, etc.
Never see a doctor without writing it down here. If you are in the hospital, write in this spot when the doctor does his rounds, if at all possible.
