This notebook is designed for use once a person has been released from a hospital setting to home care. It should go with the caregiver at all times.
Whether you are using privately hired caregivers, an agency, family or volunteer help, the Caregiver Notebook is an efficient way to assure communication, continuity of care and that any emergency information needed for the person requiring care is always available. Personalize this notebook by choosing only the components that apply to your situation.
You will need the following to set up the Caregiver Notebook
- 2” 3-ring binder.
- 3-hole-punched pocket folder
- Set of 8-tab dividers.
- Notebook paper

- Plastic protection cover
To set up notebook:
- Calendar – place in a plastic cover at the beginning of the notebook
- Make a section for:
